POSITION: Case Manager - Substance Abuse Program

                 (full-time 37.5hours/week)

LOCATION: Heartland Family Service

                  Council Bluffs, IA

SUMMARY DESCRIPTION OF WORK: 

Intensive case management responsibilities begin at this level.  Specific case management services will be provided to women in the Iowa Substance Abuse Treatment Program and their children.  Educational requirements are very specific.  Decision making, problem solving, and highly developed interpersonal skills are critical.          

PREFERRED MINIMUM QUALIFICATIONS

  • Bachelor Degree in social work or related field and 2 years experience
  • Master Degree, no experience
  • Knowledge of chemical dependency treatment
  • Valid drivers license, acceptable driving record

ACCOUNTABILITIES

  1. Demonstrates a high level of social work skills, practice and knowledge.
  2. Demonstrates good interpersonal skills in developing rapport with clients via telephone and in person.
  3. Demonstrates good interpersonal skills in developing rapport with community professional resources.
  4. Provides direct intake/assessment to identify and effectively deal with the client’s presenting problem(s)/barriers to successful completion of treatment.
  5. Provides problem solving and crisis intervention services to empower clients to develop skills necessary for a healthy/drug free family living.
  6. Demonstrates knowledge of current community programs and resources and current community human service resources.
  7. Demonstrates ability to effectively assist clients in addressing barriers by utilizing community resources.   Services to be utilized include but not limited to:  primary medical care for women, including prenatal care; and , while receiving substance abuse treatment, childcare; primary pediatric care for children, including immunizations; parenting; therapeutic interventions for children which may address developmental needs, issues of sexual and physical abuse and neglect; and case management and transportation support to assure access to needed services.
  8. Demonstrates ability to work within a team approach, along with Heartland Family Service staff and other community professionals, providing services to the family.  
  9. Able to evaluate program services and make recommendations.
  10. Responsible for compiling required program data and statistics in prescribed format necessary to comply with mandatory internal and external reporting requirements.
  11. Displays a courteous and caring attitude at all times to the clientele, volunteers, and visitors of the Agency.
  12. Cooperates and collaborates with program area staff, volunteers, and other Agency staff.
  13. Is dependable and punctual regarding scheduling and attendance.
  14. Maintains regular, prompt attendance at the office on a consistent and reliable basis.
  15. Abides by all specific program and Agency procedures, policies, and requirements.
  16. Develops personal and program related skills through participation in internal and external training   opportunities including printed material and audio and/or visual media.
  17. Performs other program related duties as assigned.
